Below you will find a listing of enhancements and fixes for CHIP.   Weekly changes are applied on Thursday evenings at 6:00 unless otherwise specified.  During updates, the system will be unavailable for approximately 15 minutes..

Updates for Thursday, Evening, January 12th, 2012

IPOS – A fixed was applied to the print version of the IPOS to the “Your Rights” section to make the supervisor a static field.

AIMS – added a comment box to the bottom of the AIMS field.

Progress Note – moved the end time entry to the signature page (SAL) in order to accommodate when the time spans midnight.

SUD Discharge Form – added date validation.

Updates for Thursday Evening, December 8th, 2011
CLS Progress Note Additions - several additional fields have been added to this document.

LOC changes - Upon discharge from the CMH admission the client’s LOC (level of care) will be reset to “NOLOC”.

SUD Admission form -
  An edit check was added to the SUD admission form  to not allow the user to enter a date of admission prior to first contact date (entered on the SARF).

Injection dispense note - The injection dispense note’s first page was modified.   For convenience the service activity log information was moved to the first page.

Diagnosis – the system was changed to reflect the diagnostic impression to be the most current diagnosis when no treating diagnosis has been completed.

Discharge summary – a fix has been applied to allow users to delete an unsigned discharge summary.

IPOS Meeting - allow an unsigned meeting to be deleted if the only thing preventing it from being deleted is a denied LOC request.

SARF and SUD Biopsychosocial Assessment - Limit the check on the diagnosis to active diangosis only.

General Documents - Fixed crashes on some documents when signing.

Updates for Sunday Evening, November 6th, 2011
Reporting EPSDT (Early Periodic Screening, Diagnosis and Testing) Services
Effective October 1, 2010, the Centers for Medicare and Medicaid Services (CMS) instructed Michigan that certain 1915(b)(3) services should be characterized as EPSDT services for individuals who were under 21 years of age on the date of service. Therefore, beginning with the FY’11 Medicaid Utilization and Net Cost Report, PIHPs must report these EPSDT services as unique units and costs in a separate column. This change does not impact reporting of encounters. On this chart, EPSDT services are noted in the column “Coverage.”

Input for IPOS - Typo in section 5, "Self Determination & Advance Directive". The term "Advance Directive" is spelled "Advanced Directive" in the first item under "Advance Directives".

Report of Death - Modify the signature section to have the following signatures:
Reporting Staff – Required
Supervisor – Required
Other Administrative Staff – If necessary (could be program director, clinical services director or a doctor)
Other Administrative Staff – If necessary (could be program director, clinical services director or a doctor)
Other Administrative Staff – If necessary (could be program director, clinical services director or a doctor)
Clinical Risk Officer – If necessary

Risk Assessment - removed the N/A field from each area of the document and made other slight modifications to the text.

Updates for Thursday Evening, September 22nd, 2011

IPOS Meeting - 9 additional signature lines have been added, when consumer has an ACT program assignment at the time the document was created.

Program Discharge Summary (MH) – added the admission date field on the discharge summary form.  The admission date auto fills with the effective date of the program assignment.

Initial/Recovery Support Plan text boxes were increased to a 2048 character limit.

SAL Update – Allow the selection of “Face to Face” and “Family Present w/o Beneficiary”, with the CPT codes H2021 and H2022. 

Respite Assessment – Under Section II page 1 - Family Situation / Resource Consideration, the printed version is slightly different from the screen version.  This has been fixed.

Updates For Wednesday Evening, May 25th, 2011

SARF changes in CHIP – Addition of Service Packages
          1.Addition of Service Package section.  Click on the “lookup” button and a list of available service package will appear.  Select the appropriate service package.  The service package setup is maintained in a different area of the system. 
          2.Click the new “Save and Release” button.  This will generate the authorizations based on the service package chosen and release the SARF.  If you click “save” it will not generate the authorization or release the SARF to the provider. 

SUD Group Progress Note - When the whole progress note did not fit on one page it spilled over to the second page, then started the next progress note right below the signature line from the first progress note.  This has been fixed.

SUD Discharge Summary - SUD Discharge Summary field not large enough and the data entered into the discharge summary section doesn’t save. This has been fixed.

SUD Discharge Summary -Need print version of SUD discharge summary. This has been fixed.

IPOS Update - Please be aware of the following scheduled changes to the IPOS meeting and Input documents in CHIP as a result of the recent MDCH audit. 
          1. A new page was added into the IPOS meeting called “Your Rights” .  
          2. A new section will be inserted into the IPOS meeting document between the Authorizations and Signatures page titled “IPOS Miscellaneous Tracking”. The intention of this section is to track the following:
               a. A copy of the IPOS has been sent/given to the client and/or parent/guardian.
               b. Adequate Notice (Medicaid/Non-Medicaid) was sent/given to client and/or parent/guardian.  
               c. Advance Directive opportunities reviewed with client and/or parent/guardian.
            3. A new page was added to the Input for IPOS between Natural Supports and Services and Support called “Self Determination &  Advance Directive”. This new section will ask if the consumer is interested in Self-Determination and additionally collect information regarding advance directives.
 

Respite Assessment Form - The Respite Assessment form has been modified to obtain more specific information for a more accurate assessment of Respite needs.

Updates For Thursday Evening, April 14th, 2011

1.PSA Update – The Folstein has been removed from page 15.

2.Program Assignments – Referral In From and Referral Out To has been added to the program assignments.

3.Employment Codes – The employment codes have been updated in CHIP to match DCH requirements.

4.Proxy Measures – The new DD Proxy Measures have been added to CHIP.

5.Report of Death Form – This form has been updated with dropdown lists for the cause of death.

6.SUD Authorizations - The ability on SUD auth to see SARF connected has been added.

Updates For Thursday Evening, March 3rd, 2011

IPOS to Accommodate The Leap Year – The IPOS requirement dates have been adjusted to accommodate the upcoming leap year.

Pre-Admission Screenings Mixed In With Crisis Screenings – Pre-admission screenings have been removed from the Crisis Screening list.

Quarter ABW Authorization Requests – The New Passages ABW program will no longer have quartered authorizations for T1017 services.

SAL Update – An error warning has been added to the signature line of the SAL to indicate that an authorization has no more units left for the requested services.

Early Termination IPOS Enhancement – When an IPOS is early terminated, the authorizations will be early terminated with the IPOS early termination date.

IPOS LOC Update – On the LOC renewal/change in the IPOS and Addendum, the LOC will default to the current level of care.

Add Additional Signatures On IPOS -  For clients with an ACT program admission, 9 additional signature lines will be added to the IPOS.

Treatment Plan Authorizations – Staff can now cancel or delete an authorization from a treatment plan as long as the plan has not yet been accepted/incorporated into the IPOS.

Login Screen Update – A new verification checkbox has been added to the login screen.

To-Do Lists – The default date range on the To-Do List screen will update to cover automatically generated to-do items.

Lock Down IPOS After Signature – Once the IPOS has been fully signed, the only way to make changes to this IPOS will be to create an addendum.
